Requesting Health Records
Federal HIPPA laws require an authorization to be completed for all medical record requests unless your provider is requesting them for your care. Because your medical record and the information it contains is confidential, we must confirm your identity before releasing your medical record. Only the patient or his/her legal guardian and/or legal representative can sign the form. You can download the authorization form below.
Authorization for Release of Protected Health Information (PHI) forms:
To obtain your health records, please bring your valid photo ID or two of the documents listed below with you to the Health Records department at 305 E Center Ave. Visalia, CA 93291. We are available to accept health record requests between the hours of 8 a.m. and 5 p.m. Monday through Friday. You can also fax the request forms with a legible copy of your ID to our fax number at 559-737-4931 for processing. We will call you when your records are ready to be
picked up.
- Authorization for Release of Protected Health Information (PHI) Form completed and signed or
- A current government issued photo ID (passport, driver’s license, State ID, or military ID) and
- Power of attorney for healthcare paperwork if the patient’s POA is requesting records.
Our turnaround times for processing are 15 days for patient requests and 30 days for all other requests.
Payments: $15.00 payable by check made out to Family HealthCare Network.
Our goal is to get your health information to the appropriate health care provider as quickly as possible; therefore, we recommend that your physician (or other healthcare provider) request a copy of your medical records to be delivered directly to his/her office by fax.
