Porterville, CA – Designed to support individuals with chronic health problems, Family HealthCare Network (FHCN) will launch a new Chronic Disease Self-Management Program in Porterville this September. Developed by the Stanford School of Medicine, the program offers attendees a unique look at managing their chronic disease. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
Subjects covered in the six week program include techniques to deal with problems such as frustration, fatigue, pain and isolation, appropriate exercise for maintaining and improving strength, flexibility and endurance, appropriate use of medications, communicating effectively with family, friends and health professionals, proper nutrition, and advice for evaluating new treatments.
Two trained leaders facilitate the workshops, one or both of whom have chronic diseases themselves. FHCN recently sent four of its Navigator Health Coaches to Stanford University to be trained as program Master Trainers.
Classes are offered to both patients and non-patients at FHCN’s Porterville Annex building, located adjacent to the Porterville Health Center, at 1137 W. Poplar Avenue. Beginning on Tuesday, September 22nd the series will run for six weeks from 2:00pm to 4:30pm.
