Stanford’s Chronic Disease Self-Management Program Kicks off New Series in Porterville

Porterville, CA – Designed to support individuals with chronic health problems, Family HealthCare Network (FHCN) will launch a new series of its Chronic Disease Self-Management Program in Porterville on March 9, 2016. Developed by the Stanford School of Medicine, the program offers attendees a unique look at managing chronic diseases like diabetes, hypertension, and chronic pain. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.

Subjects covered in the six week program include techniques to deal with problems such as frustration, fatigue, pain and isolation, appropriate exercise for maintaining and improving strength, flexibility and endurance, appropriate use of medications, communicating effectively with family, friends and health professionals, proper nutrition, and advice for evaluating new treatments. Two trained leaders facilitate the workshops, one or both of whom have chronic diseases themselves. In addition, four Navigator Health Coaches from FHCN were trained onsite at Stanford University as program Master Trainers. FHCN has offered Stanford’s model to address chronic disease management since 2001.

Classes are free of charge and are open to both patients and non-patients at FHCN’s Porterville Annex building, located adjacent to the Porterville Health Center, at 1137 W. Poplar Avenue. The six-week series begins Wednesday, March 9th, from 5:00pm to 7:30pm.
