Visalia, CA – The National Committee for Quality Assurance (NCQA) recently announced that Family HealthCare Network (FHCN) was reaffirmed as a level 3 Patient-Centered Medical Home (PCMH). In July 2012, FHCN became the first community health center in California to receive recognition by the NCQA as a level 3 PCMH. To maintain its recognition, which is valid for three years, FHCN demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. The maximum level granted by the NCQA, FHCN’s 13 health centers were recognized for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.
“We are very proud of our team for achieving this prestigious recognition. There are very few health centers in the country that can say they are a level 3 Patient-Centered Medical Home,” stated Kerry Hydash, Family HealthCare Network President & CEO. “Our team is continually working together to raise the bar in how we provide high-quality care and increase access for our patients. Ultimately this is about improving care for our patients.”
The NCQA PCMH is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care, and reduce costs. As a medical home, FHCN fosters ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes like FHCN can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.
