Family HealthCare Network is a Federally Qualified Health Center (FQHC). Family HealthCare Network provides quality, integrated primary health care services to all community members who live in Family HealthCare Network’s service area.

    How do I make an appointment at Family HealthCare Network?

    Family HealthCare Network operates 36 Community Health Centers. To schedule an appointment as a new or existing patient, please follow these steps: Step 1: Contact our Call Center to schedule an appointment. The Tulare/Kings Call Center is open Monday through Friday, 7:00am to 9:00pm and Saturday – Sunday, 8:00am – 5:00pm. The Fresno Call Center is open Monday through Friday 8:00am to 5:00pm. Call our Navigation Call Center at 877-960-3426 (FHCN) – Tulare/Kings or 866-324-6012 – Fresno and select a health center that meets your needs. Step 2: A Call Center Navigator will schedule an appointment for you with a provider of your choosing who best meets your health care needs. Step 3: On the day of your appointment, please bring the following documentation to complete your registration visit:

    • Insurance card-for patients who have insurance
    • Proof of income
    • Proof of address
    • Identification such as a driver’s license, passport or other photo identification-for patients who have these documents.

    Step 4: Selecting your Primary Care Provider (PCP) and Provider Team Family HealthCare Network is a Patient-Centered Medical Home. At your visit, you will be asked to select a Primary Care Provider (PCP). You will also have access to a health care team to support coordinating your health care needs with a focus on prevention and wellness. Your team will include your Primary Care Provider, a mid-level provider (physician assistant or nurse practitioner), a medical assistant, health educator, a behavioral health provider, a dietician, and other health professionals such as a patient navigator to support your health care goals. When needed, your team will help coordinate your care including referrals, consultations, imaging, and hospitalizations.

    How do I cancel an appointment at Family HealthCare Network?

    Call your health center at 877-960-3426 (FHCN) – Tulare Kings or 866-324-6012 – Fresno and a Call Center Navigator will help you cancel your appointment. Please call 24 hours in advance for cancellations.

      What should I do if I need an appointment right away?

      Call your health center at 877-960-3426 (FHCN) – Tulare/Kings or 866-324-6012 – Fresno and a Call Center Navigator will assist you in scheduling a same-day appointment. If you need to be seen after hours, please call Family HealthCare Network’s Call Center and you will be connected with Family HealthCare Network’s 24-hour Nurse Triage Service. If you have an Emergency, dial 911.

      How do I pay for services at Family HealthCare Network?

      We accept all Covered CA Medical Plans, Medi-Cal, Medical Managed Care, Medicare, most private insurance plans, and we offer a Sliding Fee Scale. If you are uninsured, Family HealthCare Network provides a Sliding Fee Scale that is based on your family size and family income. A Family HealthCare Network representative will assist you to determine the co-pay that best meets your financial needs. The Sliding Fee Scale makes health care affordable for you and your family.
