Family HealthCare Network (FHCN) is a nonprofit community-based organization that operates 32 sites, 27 of which are Federally Qualified Health Centers (FQHC) throughout Tulare, Kings, and Fresno Counties. The organization is governed by a volunteer Board of Directors and has developed a comprehensive, patient-centered health care delivery model with sites in Tulare, Kings, and Fresno Counties. Health center sites are located in Cutler-Orosi, Dinuba, Farmersville, Fresno, Goshen, Hanford, Ivanhoe, Pixley, Porterville, Springville Strathmore, Terra Bella, Three Rivers, Traver, Tulare, Visalia, and Woodlake.
Family HealthCare Network also operates a Mobile Health Center program, the Ventanilla de Salud Resource Center located in the Fresno Mexican Consulate, and the Porterville Resource Center in the community of Porterville.
Family HealthCare Network has grown from a small outpatient health center in Porterville serving the farmworker community to a multi-site primary health care network serving 253,850 patients in Tulare, Kings, and Fresno Counties. We are one of the largest providers of care in the country to the underserved population and stand as a national model in demonstrating value and effectiveness in reducing health disparities and improving health status in communities in need.
Family HealthCare Network’s patient-centered delivery model offers a full array of coordinated services to meet the needs of an entire family. We provide women’s health services including family medicine, obstetrics and gynecology, pediatrics, adult and children’s dentistry, internal medicine, behavioral health, pharmacy, optometry, chiropractic care, podiatry, general surgery consultation, perinatology, vascular surgery, nutritional counseling, case management, community health, eligibility assistance, translation/interpretation services, imaging services, clinical laboratory, telemedicine such as digital retinopathy screening, and transportation.
Family HealthCare Network has earned a reputation for excellence nationally and statewide. We are dually recognized by The Joint Commission for demonstrating compliance with The Joint Commission’s high standards for health care quality and safety, and certified with The Joint Commission’s Gold Seal of Approval® as a Primary Care Medical Home. Family HealthCare Network is one of an elite group of health care providers in the nation to earn this national recognition for quality, safety, and excellence.
The National Committee for Quality Assurance (NCQA) has also recognized Family HealthCare Network as a Level-Three Patient Centered Medical Health Home. This honor recognizes that our health care teams work in partnership with patients to provide care that’s coordinated, effective and timely. This is the highest recognition level awarded by the Committee and only a handful of community health centers have attained this recognition across the nation.
Family HealthCare Network has been recognized for the integration of technology into its health care delivery approach. We were the first California health center network awarded for the achievement of “meaningful use” in 2013.
Family HealthCare Network serves as the only Community Campus in California for students from A.T. Still University’s (ATSU) School of Osteopathic Medicine based in Mesa, Arizona. A.T. Still University students are embedded at the organization’s health centers where they study, train, and complete their clinical rotations during their last three years of medical school. Family HealthCare Network extended its partnership with the university and has established a Physician Assistant Program. Family HealthCare Network also maintains a partnership with New York University Lutheran Medical Center Dental, based in Brooklyn, New York that establishes a dental residency program at FHCN health centers. The dental residency program is a 1-year advanced training program with the option of a second year extension and places dental residents in community health center sites for comprehensive general dentistry training. In 2015, FHCN received accreditation from the Commission on Dental Accreditation (CODA) as a clinical training site for NYU Lutheran Medical Center Dental Program.
Family HealthCare Network’s partnership with the University of California San Francisco (UCSF) Fresno Medical Education Program brings residents and students from a world-class university to the valley offering emergency medicine, family and community medicine, internal medicine, obstetrics/gynecology, orthopedic surgery, pediatrics, and surgery. UCSF Fresno gives valley residents access to the highest quality health care services while enabling doctors in training to work alongside amazing physicians and experience the dynamic interplay of scholarship, research, patient care, and health.
