We provide services to our patients in their communities and in their preferred language. We make a short-term impact by helping our patients avoid episodic emergency room treatment, and a long-term impact by providing a health home that is centered and coordinated for our patients, protecting and preserving their health for the future. Primary Care Clinics are required to submit an Annual Utilization Report of Primary Care Clinics by February 15th each year for the prior calendar year. Below you can find a link to the State of California Office of Statewide Health Planning and Development website where you can find those reports for our specific centers.
Family HealthCare Network is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). We offer services without regard to religion, race, color, national origin, disability, age, sex, and number of pregnancies, and do not impose a duration residence requirement, nor do we require that the client be referred by a physician.
This health center receives Health and Human Services funding and has Federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
