Family HealthCare Network was the first freestanding ambulatory health care center in Tulare County to be accredited by the Joint Commission on Accreditation of Healthcare Organizations!

Nutrition services provided at Family HealthCare Network include education and counseling programs for both individual patients and in a group setting. Educational information is provided and nutritional care plans are developed for special dietary needs such as obesity, hypertension, diabetes as well as pregnancy.

FHCN’s nutritionists will coordinate with clinical, health education, community health and other staff members in the provision of service to patients. The integration of Nutrition services within FHCN’s overall system will help ensure that each patient receives coordinated care for all their health problems and needs.

Nutrition services are currently offered at the following health centers:

  Visalia Bridge  
Visalia Oak Cutler/Orosi Porterville
Three Rivers Woodlake Ivanhoe

 

Family HealthCare Network currently has 13 locations throughout Tulare County of which 11 are clinical sites. Family HealthCare Network is a community-based organization governed by a Board of Directors, which is composed of community representatives and patients consumers.
