Health Education
Health Education services provided at Family HealthCare Network include education and counseling programs for both individual patients and in group settings. Health Education is provided using methods that are culturally and linguistically appropriate. Health Educators and Health Promotions Instructors provide Health Education relating, but not limited to, Asthma, Breastfeeding, Childbirth, Diabetes, Family Planning, Hypertension, Hysterectomy, Lead Poisoning, Lower Back Pain, Menopause, Parenting, Pesticides, Sexually Transmitted Infections. Additionally, Health Promotion Instructors provide prenatal Health Education to expecting mothers through the Comprehensive Perinatal Services Program.
Community Outreach
Family HealthCare Network respects the People of the Communities that it serves. As such, Family HealthCare Network has Community Services Representatives who are from the Communities and who act as liaisons to ensure that Peoples voices are heard at Family HealthCare network. Additionally, the Community Services Representatives are great referral resources to patients who have issues relating to shelter, food, clothing, and other basic human needs. They are all culturally and linguistically sensitive to the needs of the Community. Additionally, Community Services have been trained to assist patients with filling out various forms to such programs as AIM, Healthy Families, and Medical. Community Services are offered at all our centers.
Transportation
In order to ensure access to primary care services, Family HealthCare Network provides transportation services to our patients that experience transportation barriers. FamilyHealthCare Network is committed to assist patients who have no transportation and would otherwise be unable to attend their medical appointments. Transportation is provided Monday – Friday either door-to-door or by an established route. Family HealthCare Network currently provides transportation service to more than fifteen communities throughout Tulare County.
Billing Information
Patients are offered a variety of payment options including commercial insurance, Medicare, Medi-Cal, CHAMPUS, Workers Compensation, and self-payment on a sliding fee scale according to family size and income. Family HealthCare Network contracts with most major Preferred Provider Organizations (PPOs) for services. You are advised to always check with your insurance plan to confirm if Family HealthCare Network is eligible to use your individual health plan. Assistance in completing applications for Healthy Families, AIM and Medi-Cal is also available at all of our health center locations.
Patient Billing
