You may make an appointment by calling your clinic—see the numbers below:
You may also make an appointment by stopping by your clinic during regular clinic hours.
Urgent Medical Needs
To make a same-day appointment, please call your clinic as soon as possible after 7:00 a.m. on the day you need to be seen. You may also ask if any providers have openings in their daily schedules. Walk-ins are welcome and are served on a first come first serve basis, but priority is given to patients requiring immediate care. After regular business hours, call your clinic in order to speak to the on-call provider.
Cancelling an Appointment
It is very important that you call to cancel at least 24 hours ahead of time if you cannot come to your scheduled appointment. We hold your appointment time especially for you. If you can’t give us 24 hour notice, please call as soon as you can to cancel. When you miss an appointment without letting us know, we are unable to use this time to help other patients. We track no-shows (missed appointments without being cancelled) and multiple no-shows can result in restrictions for scheduling future appointments.
About Your Appointment
When you come to your appointment, it is important to always bring the following items:
- Written permission from a parent or guardian if a child is brought in by someone other than the legal guardian;
- Your insurance card;
- A list of your current medications (bring the bottles in themselves if you are unsure);
- For children’s visits, please bring our child’s immunization records;
- A list of questions you want to discuss with your provider. If you have many questions, your provider will select those that are the most important and make a follow-up appointment to address the rest;
- Your co-payment. This will be collected at the time of your visit.
