Who We Serve

We welcome everyone to Family HealthCare Network and although we have grown, we have remained committed to ensuring that we provide quality care to everyone in the communities we serve. 

Our patients are from all “walks of life,” and since 1976 have been the health home for individuals and families that have lacked access to any other source of health care, because of financial, linguistic, cultural, geographic or transportation barriers. Overall, 75 percent of our patients are of Hispanic origin and 42 percent speak a language other than English. Most are families, with 78 percent of them living below 100 percent federal poverty line. The majority are young and female. A significant portion of our patients receive Medi-Cal; 25 percent have no insurance and care is provided to patients on a sliding fee scale basis.

 

The statistics tell only part of the story. It is how we serve our patients that sets us apart.

 

We provide services to our patients in their communities and in their language. We make a short-term impact by helping our patients avoid episodic emergency room treatment, and a long-term impact by providing a health home that is centered and coordinated for our patients, protecting and preserving their health for the future. Family HealthCare Network’s phenomenal growth and success are a testament to the importance of our mission: quality care for all.

Primary Care Clinics are required to submit an Annual Utilization Report of Primary Care Clinics by February 15th each year for the prior calendar year.
