Insurance Benefits

Health Insurance
Health Insurance Is provided at no cost to the employee.  The plan is a Preferred Provider Organization (PPO) administered thru Advantek.  Prescription coverage is administered thru BeneCard.

Dental Group Insurance
Dental Insurance Is provided at no cost to the employee. The plan is administered thru Premier Access Insurance Company, a national carrier and widely accepted dental plan.  The plan offers 3 Tiers of benefits that you can choose from.  Orthodontia is available with no waiting period for children up to age 19.

Eye Care Insurance
Eye Care Insurance Is provided thru Ameritas Group at no cost to for employee coverage.  The frequency allowance for an eye exam is 12 months, Lens 12 months, and frames 24 months.  See the highlights sheet enclosed in your packet for more details.

Group Term Life Insurance
The benefit payable to beneficiaries under the Group Term Life Insurance for employees is equal to 150% of annual earnings rounded to the next higher $1,000, unless already an even $1,000.  The maximum amount is $150,000.  Dependent Life Insurance Coverage for Spouse is $5,000.00 and for Children 6 months to age 19 (26 in school) is $2,000.00. Except that, such amount will be $1,000.00 for dependent children from live birth to 6 months of age.

Long Term Disability
The benefit payable to you for each full month of a Total Disability Payment Period will be the smaller of: 66 2/3% of your covered monthly compensation at the time of the total disability.  This benefit is at no cost to the employee.  There is a 90-day waiting period for Long Term Disability coverage, which must be accumulated within a 360-day period.

Accidental Death & Dismemberment Insurance (AD&D)
The amount of insurance is equal to the amount of insurance provided for Group Term Life Insurance. You must be insured for Life Insurance to be eligible for (AD&D) benefits.

Profit Sharing Plan
Provided to employees who have attained age 21 and who have completed a year of service as defined by the plan.  Within budgetary constraints, an annual contribution equal to 5 percent Network profitability is contributed to the plan in the employee's name and vesting of the employee in the plan takes place over a five-year period.

Staff Incentive Plan
IDesigned to meet the needs of the organization while, at the same time, meeting the short and long-term individualized needs of staff.  The Staff Incentive is based on both productivity and participation in a team setting.  To participate in the Incentive you must meet the requirements.

Direct Deposit
Provided to all employees at no cost to the employee.  Your direct deposit will take two pay periods to be effective and you may have up to three (3) accounts.  Employees can start a direct deposit at any time and make changes through out the year.

ADDITIONAL BENEFIT OPTIONS
These Plans are offered to you on a voluntary basis, deductions are set up thru payroll for your convenience.  The additional benefits premiums are the employee’s responsibility.

You can voluntarily participate in the Supplemental Plans.  The plan allows you to post-tax insurance coverage on the plans below:

  • Critical Injury Insurance with Cancer rider
  • Voluntary Life Insurance

FSA’s:  Health Care Flexible Spending Account will help you to pay out-of-pocket medical costs expenses while increasing your spendable income.  This is a pre-tax plan and you can elect minimum coverage of $250 to a maximum of $3000 annually.
Example $250.00 divided by 24 = $10.41 per pay period or $3000 divided by 24 = $125.00 per pay period.

The Dependent Care Account is also pre-tax and you can elect minimum coverage of $250 to a maximum of $5,000 annually for child care expenses with a license care provider.   
Example $250.00 divided by 24 = $10.41 per pay period or $5000 divided by 24 = $208.33.00 per pay period.  Examples are based on a calendar year from January thru December.
You may elect to enroll during New Hire Orientation and annually during Open Enrollment in November and Dencember.  These plans are based on a calendar year from January thru December.
The FSA accounts need to be renew annually, this benefits is not renewable by your employer.

The “USE IT OR LOSE IT” rule for the Flex Spending Accounts (FSA):  If you contribute dollars to a flexible spending account and you do not use all of the monies you deposit, during the plan year (Jan thru Dec) you will lose any remaining balance in the account at the end of the plan year.  A very important thing to member….the rule exists because the IRS has established strict guidelines on the plans with tax advantages.

TAX SHELTERED ANNUITIES (TSA’S): Family HealthCare Network sponsors TSA investments programs in which employees can choose from.  This plan is employee sponsor and deductions are set up thru payroll.   A TSA’s election can start at any time with minimum contributions of $50.00 per pay period to a maximum contribution of $14,000.00 annual. 
Example:  $50.00 X 24 = $1200.00 or $625.00 X 24 = $15,000.00.
NOTE:  If you are over 50 years old you may choose to do the Catch-Up-Plan.
ENROLLMENT:  You can enroll, decrease or increase your contributions anytime during the year.

TERM LIFE INSURANCE: is provided for employee and dependents at a low cost, you can elect the minimum coverage of $10,000 and up to 5 times your annual salary.  Dependent coverage for spouse is 50% of your elected coverage and your minor children up to $10,000 of coverage with a minimum of $20,000 coverage for employee.
Enrollment:  During New Hire Orientation and annually during the month of February

WORKERS’ COMP : provides medical coverage for any injury or illness caused by your job.
