Our mission is simple…

To provide quality health care to everyone in the communities we serve. Our approach offers services that are centered on you and your family, and coordinated by a health care team to address your health care needs. Our services are recognized nationally for quality, safety, timeliness, and coordination. We are proud of our history serving in local communities and providing exceptional services that are sensitive to patient needs. At Family HealthCare Network, caring about you and our community is our most important job because your health is our mission.

Family HealthCare Network (FHCN) is a nonprofit community-based organization that operates 18 federally qualified health centers (FQHC) throughout Tulare and Kings Counties. Our mission is “to provide quality health care to everyone in the communities we serve.” The organization is governed by a volunteer Board of Directors and has developed a comprehensive, patient-centered health care delivery model with 22 total sites in Tulare, Kings, and Fresno Counties. Health center sites are located in Cutler-Orosi, Farmersville, Goshen, Hanford, Ivanhoe, Pixley, Porterville, Springville, Terra Bella, Three Rivers, Traver, Tulare, Woodlake, and Visalia.

FHCN also operates the Ventanilla de Salud Resource Center located in the Fresno Mexican Consulate and a Community Resource Building in the community of Porterville.

Family HealthCare Network has grown from a small outpatient health center in Porterville serving the farm-working community to a multi-site primary health care network serving 218,337 patients in Tulare and Kings Counties. We are one of the largest providers of care in the country to the underserved population and stand as a national model demonstrating value and effectiveness in reducing health disparities and improving health status of communities in need.

Family HealthCare Network’s patient-centered delivery model offers a full array of coordinated services to meet the needs of an entire family. We provide health services including family medicine, obstetrics and gynecology, pediatrics, adult and children’s dentistry, internal medicine, behavioral health, pharmacy, optometry, chiropractic care, podiatry, vascular surgery, general surgery consultation, vascular surgery, perinatology, nutritional counseling, health education, case management, community health, eligibility assistance, translation/interpretation services, imaging services, clinical laboratory, telemedicine such as digital retinopathy screening, and transportation.

Family HealthCare Network has earned a reputation for excellence nationally and statewide. We are dually recognized by The Joint Commission for demonstrating compliance with The Joint Commission’s high standards for health care quality and safety, and certified with The Joint Commission’s Gold Seal of Approval® as a Primary Care Medical Home. Family HealthCare Network is one of an elite group of health care providers in the nation to earn this national recognition for quality, safety, and excellence.

The National Committee for Quality Assurance (NCQA) has also recognized Family HealthCare Network as a Level Three Patient-Centered Medical Home. This honor recognizes that our health care teams work in partnership with patients to provide care that’s coordinated, effective, and timely. This is the highest recognition level awarded by the Committee, and is one that only a handful of Community Health Centers across the nation have attained.

Family HealthCare Network has been recognized for its integration of technology into its health care delivery approach. We were the first California health center awarded for the achievement of “meaningful use” in 2011. We have continued to advance our technology by implementing electronic record systems throughout our network to improve patient needs and outcomes.

Family HealthCare Network serves as the only Community Campus in California for students from A.T. Still University’s School of Osteopathic Medicine based in Mesa, AZ. ATSU students are embedded at the organization’s health centers where they study, train, and complete their clinical rotations during their last three years of medical school. Family HealthCare Network extended its partnership with the university and has launched a Physician Assistant Program. Since 2008, 70 individuals have graduated from the program.

Family HealthCare Network also maintains a partnership with Lutheran Medical Center Dental (LMC Dental) Program based out of Brooklyn, NY that establishes a dental residency program at Family HealthCare Network health centers. The dental residency program is a 1-year advanced training program with the option of a second year extension and places dental residents in Community Health Center sites for comprehensive general dentistry training. LMC Dental has pioneered the innovative concept of a Community Health Center (CHC) center-based postdoctoral dental residency program and is now the largest of its kind.

For 40 years, Family HealthCare Network has advanced the development of community-based primary health care delivery systems to address the health care needs of underserved and vulnerable populations. We are community driven and committed to improving the quality of life in our communities served.
